How much does it cost to get a DBQ completed?
Our standard fee for a DBQ service is $300 per DBQ.
How do I know if the provider is qualified?
All of our providers are licensed medical professionals in their respective state with experience in completing DBQs. We carefully vet each provider to ensure they meet our high standards of care and professionalism.
What if I need to reschedule my appointment?
You can reschedule your appointment by logging into your account on our website and selecting a new time that works for you. Please try to reschedule at least 24 hours in advance. Please note that there is a $50 "no-show" fee if you miss your appointment without prior notice.
What should I bring to my appointment?
